I am writing this letter to refer to your care Ms kylie who has been diagnosed with myocardial infarction for further management and care.
Upon admission, Ms kylie expressed discomfort and heaviness in her forearms. An x-ray demonstrated ST-segment elevation hence myocardial infraction was elicited. Subsequently, she underwent an emergency angioplasty. postoperatively, her pain had been managed but she seemed confused and fearful regarding her diagnosis. please note that she is on atorvastatin, cilazapril, metroptopol.
Pertinent medical history includes osteoarthritis, mild asthma and dyslipidaemia. Socially, she is married and has one daughter.
As per the discharge plan, it would be greatly appreciated if you could adhere to the physiotherapy regime and monitor her recommended medicines along with her diet. Kindly arrange an occupational therapist and a social worker to educate her to cope up with daily activities. Please assess her eligibility for sickness allowance as well
