I am writing to request a follow-up for Ms Jane who needs further assessment and management for sexually transmitted diseases and blood born viruses along with a discussion of possible contraceptive methods. Initially, on 18 th September 2008 she presented with concerns regarding her weight gain which is suspected due to the contraceptive effect and requested a Paps smear test. Consequently, the test was taken. On her subsequent visit three days later, she and her partners have been informed regarding her positive test result for chlamydia. she has been commenced on azithromycin and advised not to have sexual contact after a week of treatment. Socially, she has the habit of smoking 15 cigarettes a day and drinks 2 glasses of wine on weekends. Her sexual history reveals that she is not sure about monogamous sexual practice and had sexual contact with her new partner 14 days ago. Kindly note that she is on oral contraceptive pills for the last 12 months. It would be appreciated if you could educate regarding contraceptive methods.
