DATE & TIME OF CANCELLED CLASSES: 3 Dec 2021 17: 00 - 17: 25 4 Dec 2021 10: 00 - 10: 25 5 Dec 2021 07: 00 - 07: 25 5 Dec 2021 07: 30 - 07: 55 5 Dec 2021 08: 00 - 08: 25 5 Dec 2021 20: 30 - 20: 55 In spite of the emergency matter at hand, I sent a notification to CT-Son via Skype first and foremost to avoid complications with Sankei Online en route from my house to the hospital on December 3, 2021, at 6: 21 AM. Hereto attached is the screenshot with my conversation with CT-Son as ANNEX A. On December 3, 2021, my 2 years old and 10 months daughter named Pearlyzza Gold Ponce became acutely ill with fever, diarrhea and vomiting. The birth certificate of my daughter is hereby affix as ANNEX B. Consequently, in my capacity as a mother rushed her to the nearest healthcare facility. Proof of her hospital entry is reflected as ANNEX C. From December 3 to 6, 2021, my toddler was hospitalized as an in-patient for 3 days and 3 nights to combat dehydration. That prompted me to spend sleepless nights with her at the facility to aid with her recovery. On December 6, 2021, three days later, the pediatrician discharged my kid from hospital. The aforesaid incident caused the cancellation of six classes. Official Receipt of the hospital bill is hereby affixed as ANNEX D. Originally, my mother personally took care of my toddler as I am a solo parent. However, she was also suffering from diarrhea two (2) days prior to the hospitalization of my little girl. That leave me no choice but to deal with the situation directly. Fortunately, my mother has finally fully recovered on December 10, 2021. Hence been now able to take care of my daughter again. Revitalizing work and motherhood have never been easy. That’s why, I am grateful for the adequate childcare provision from my mother. Without her assistance, I could not find a means of living both online and offline. I have never contracted any marriage with the biological father of my child. Since our relationship was terminated prior to the birth to our baby, he has neither communicated with us nor extended any financial support whatsoever. I have been solely supporting my little one since his abandonment in 2019. I tried to communicate with the CT on-duty during my stay in hospital as can be seen in ANNEX E. Kindly excuse my absence. Now that my mother and my toddler are completely healed, I am ready to work again. I humbly beseech you to please lift my suspension. I remain at your disposal for any further information.
