I am writing this letter to refer the above-mentioned patient to your facility for further care and management as he has been diagnosed with stage 3 carcinoma of the breast.
Mr Anthony was admitted to the emergency department on 23/5/2017 with ulcerated, haemorrhaging right anterior chest mass. Ct chest revealed the presence of a soft tissue mass in the right chest wall. Hence, an incision biopsy of the right breast mass has been performed. Also, he has been commenced on hormone therapy with tamoxifen 20 mg daily with one course of palliative radiation.
Upon evaluation, doctors found him unfit for chemotherapy or curative treatment. Hence, hospice care was recommended after consulting with his son. Socially, he lives with his wife. Both suffer from dementia so it would be appreciated if you could inform the availability of a hospital bed for her admission as well.
