Dear jenny,
This letter will inform you regarding Mr Derek who underwent coronary artery bypass grafts who needs dietary advice for further management.
Initially, he presented to us on 20th December 2017 with complaints of pain in the chest which started about five months ago and worsened on excretion, tightness in the chest, heaviness in both arms and shortness of breath. Subsequently, he has been diagnosed with obstructive coronary artery disease hence the surgery. His postoperative period was uneventful but had difficulty in defecating which resolved after a day. He has been commenced on physiotherapy and advised to consume a low-fat diet.
His medical history reveals that he has constipation occasionally for which he is taking isobgol.
Socially, He has the habit of smoking five to six cigarettes and consumes 600ml of beer per day.
His dietary habit consists of consuming sausages, fried chips, pizzas, and pasta. His BMI is 29 which is remarkably high.
As per the discharge plan, it would be greatly appreciated if you could help him to reduce his weight and educate him regarding a low-fat diet along with simple recipes that can be prepared at home. Kindly encourage him to decrease alcohol consumption and quit smoking. Please note that he is on aspirin and vicodine.
Kindly send the detailed guideline to his home address. Thank you in anticipation for agreeing to help Ms juicy. Please contact me if you have any queries regarding Ms juicy.
Thank you
