Dear Dr. Vincent Raymond
Re: Dulcie Wood (DOB: 15. 07. 1946)
I would appriciate visiting Dulcie Wood as a new patient to your practice, who was diagnosed with cardiac flutter. Although treatment with Noten was initiated and its dosage was increased, she reported increase in the episodes of her heart flutter sensation. So, I decided to refer her to your practice for further assessments.
Her symptoms have initiated a few months ago. At first appointment, she was taking Zocer 20 mg daily and Calcium Caltrate 1 daily. So, Noten 50 gm half of tablet daily in morning was added to her medications due to oneset of heart flutter sensation. Her blood pressure was 145/75 on that time. At the next visit, although the heart fluttering sensations decreased, she reported 2 times waking up at night due to heart flutter in the last couple of weeks. Thus, the Noten dosage was doubled by addition of half of tablet in pm to her prescription. Inspite of slight improvement, she mentioned increase of heart flutter episodes during last three days which was sensed at both day and night. Also, I detected significant increase in the patient’s blood pressure (180/90) in this visit which can be related to her anxiety.
Therefore, I booked an appointment for Mrs. Wood by contacting your receptionist. I should mention the patient herself recommended you as you are her sister doctor.
