Dear Sir, I am writing this letter to requesting you about grant me medical leave for one month. My name is Kosha Thakkar and I am working in a design department at morning shift. I am suffering from physical disease called gastric ulcer and high fever. Doctor has prescribed medication and an advice to take plenty of rest. As I have to visit hospital every two days; I am not able to come at work regularly on the contract it can cause higher infection spreading. The design editing and approval can be managed by staying at home. I will be in constant Contact with the team member and make sure that assignment will be complete within deadline.
