Dear Sir or Mam, I am ABC from kitchener and was admitted in your hospital from last few days after suffering from a migrane attack. Through this letter I want to make a request as I have forgotten my wallet and a diary in the room no. 786. Yesterday while checking out of the hospital, I didnot realised my belongings were missing until I reached my house travelling 100 kms.
