To,
The Manager,
ABC bank's Head office,
Dear Sir,
I am Ms. Shah presently residing in Sector 24, New Delhi. I also hold an account with your bank for 20 years now. Recently, I had applied for an additional bank card to enjoy some liberty with my credit card expenses. However, I am writing this letter to you because when I received an envelope yesterday, it did not contain the card in it. Moreover, it had all relevant documents related to activation process and password. Therefore, I am afraid about the possibility for the card to be mishandled by someone from the bank staff or courier agency which delivered it. Consequently, it can be misused to any extent and lead to unforeseen situations.
Hence, I would like to block it from your database itself. Fortunately, the papers which are received by me contain the credit card number and expiry date, which would be enough for the deactivation. Along with this letter, I have enclosed my signed consent form which would be required by bank staff for further process. Apart from this, I insist you to investigate this matter and identify the reason behind such a mishap.
Sincerely,
Ms. Shah.
