Letter to the manager of supermarket to complain on minor accident

Dear sir, I am corresponding this letter with regard that i reside nearby this market, where 21st Nov on Monday nearly 7pm i went to this site with my sister in law, who is pregnant, just want to purchase baby milk and prescribed medicines from this location. After taking our required things as we cross the corner of the pharmacy, however the road was quite muddy and slippery, further the whole antity was dark, even can not recognise the far away person. By the low fact, a motor cyclist came in front of me, indeed unexpected wreckage to me, as a result i could not control myself due to slippery place, so badly fell down. Though i got minor injury, but rider was also embaressed on his act, although he was not responsible in real. The long story short, It was all about your neglegence, whether you should regularly check your street lights or change those whilst are not working, more overyou should repair the damage portion of the way which is messy and wet.
