Dear Dr Anderson,
Re: Mr. Daniel McCrae, DOB: 17. 10. 1962
I am writing to refer Mr. McCrae, a 52-year-old barrister. I would be grateful if you could access him for adenocarcinoma of ascending colon as soon as possible.
Mr. McCrae is a married man who lives with 4 children. He is a smoker and his BMI is 28. 1.
Initially, he presented to me on 19. 09. 13 with signs and symptoms suggestive of viral infection. Therefore, I prescribed him paracetamol and recommended to take rest for 3 days until symptoms disappeared.
On 08. 02. 14, he visited me again as he was feeling tired and he had never totally recovered from previous infection. He also complaint of abdominal discomfort, diarrhoea alternating with constipation and fatigue. However, he did not have the family history of colorectal carcinoma, colonic polyps or inflammatory bowel disease. All the examinations were normal.
Today, he comes back with test results which revealed reduced Hb (91), increased Hct (34%), positive FOBT and abnormal colonoscopy. Therefore, biopsy was taken and adenocarcinoma was diagnosed.
I would greatly appreciate your further assessment and ongoing management of this patient. If you require any further information, please do not hesitate to contact me.
Yours sincerely,
Doctor
